APPLICATION PACKET

We have added a link (below) to download our scholarship application packet.  The information required is extensive in nature and must be filled out completely with supporting information provided where applicable.  Once an application deadline has been set, all applications must be postmarked by this date or be disregarded.  We apologize in advance for strict enforcement of this policy.


The Jackie Spellman Scholarship is available to U.S. Citizens who are high school seniors, community college students or four-year university students who are leukemia or lymphoma survivors, further defined as those individuals diagnosed with leukemia or lymphoma, as well as those individuals whose family members have had or have leukemia or lymphoma while part of the same household.  Applicants must meet all criteria established within the application process.

*Additionally, each scholarship awarded to a recipient is awarded for undergraduate studies at an accredited college or university, as defined in section 170(b)(1)(A)(ii).  The scholarship funds will be paid directly to the college or university for tuition and fees.  Any unused funds must be distributed back to the Foundation.
